Specifications include, but are not limited to: The selected firm will provide, at a minimum, the services listed below within their scope of professional practice standards. 1. Health risk assessments, biometric screenings, and Comprehensive Health Review and reporting these activities (not results) to CPS Benefits Team for the purposes of issuing Wellness Credits for reduced premiums. 2. Health coaching using scientifically proven behavioral change methodology 3. Preventive health services 4. Behavioral Health Services (provide 2-3 on-site counselors) 5. Dietician or Weight Management Services 6. Provision of primary care services to employees, retirees and dependents who choose to use the clinic for that purpose, as a possible alternative. 7. Episodic care 8. Chronic disease management utilizing evidence-based guidelines 9. Lab services 10. Injury treatment 11. Wellness and preventive care, to include collateral material and counseling to improve employee health and to assist in managing chronic disease 12. Allergy injections, as prescribed by authorized health care providers 13. Flu and other immunization vaccinations (both preventive and those required as a result of the individual’s job description) 14. Maintain patient health records consistent with all applicable State and Federal laws, to include relevant privacy regulations and HIPAA guidelines 15. Maintain open and frequent communication with CPS’s Employee Benefits Management representatives regarding observable trends 16. Access to tools and wellness content to be used by participating CPS employees who are engaged in the onsite Health Coaching program and have internet access 17. Comprehensive integration with carrier and Onsite retail pharmacy Vendor 18. Provide Employee Physicals as required (Currently required for Transportation/Bus Driver Employees) 19. The ability to integrate with Anthem and Onsite Rx at the level of the EMR (Electronic Medical Records) plus future carrier
