The Successful Offeror shall provide the following services: 1. Assessment and diagnosis. 2. Prescribe medications for SUD. 3. Treat Hepatitis C in conjunction with treating SUD. 4. Participate collaboratively with all treatment team members in making treatment decisions based on patient need. 5. Work closely with peers, clinicians, nursing staff and other prescribers to service patients. 6. Complete documentation within the same day as service is provided in the HAMHDS’s Electronic Health Record. 7. Be available for consultation outside of normal work hours (8:00 AM – 4:30 PM, Monday through Friday), but during HAMHDS hours of operation (until 7:30 pm Monday through Thursday) to address medication concerns. 8. Provide services in multiple locations including the mobile unit in Henrico, Charles City and New Kent Counties. 9. Provide services in person and on occasion by telehealth. The vast majority of services are to be provided on site. 10. Be available for education to providers/staff/clients on treating SUD
