Specifications include, but are not limited to: 1. The Contractor shall furnish all labor, supervision, supplies, and equipment as necessary, to provide the services listed in this contract. 2. Provide Daily Services five (5) days per week, Monday through Friday, between the hours of 7:00 am and 3:30 pm. Services will not be required on Commonwealth of Virginia State holidays which include: New Year's Day, Martin Luther King Day, Washington's Birthday, Memorial Day, Juneteenth, Independence Day, Labor Day, Columbus Day, Voting Day, Veteran’s Day, Thanksgiving Day, Friday after Thanksgiving, Christmas Day, and any other days deemed a state holiday or state emergencies by the Governor (see the Department of Human Resource Management at http://www.dhrm.virginia.gov/payandholidaycalendar). When a holiday(s), inclement weather, delayed openings, early closings, or other reasons, beyond the Agency’s control, that services cannot be rendered, the Contractor shall deduct a pro-rated amount from the monthly invoice, based on the cost per month, divided by the number of working days in that month, for each day, the service is not provided. 3. LEVEL OF SERVICE: The Contractor shall have experience in cleaning medical office and clinic facilities of similar size and shall provide a level of cleaning that is at least equal to standards defined by the Centers for Disease Control and Prevention Environmental Cleaning Procedures, Environmental Cleaning Procedures | Environmental Cleaning in Global Healthcare Settings | HAI | CDC and Per OSHA cleaning and disinfecting guidelines, CDC's_Cleaning_and_Disinfecting_Guidance.pdf (osha.gov) 4. CONTRACTOR-FURNISHED EQUIPMENT AND SUPPLIES: A. Equipment: The Contractor shall furnish all janitorial equipment required to perform under this contract. Limited space will be provided for all equipment supplied by the Contractor which remains on the job site during the life of the contract. The equipment must be maintained in good operating condition and in sufficient quantities to adequately perform all services and available to the Contractor's employees at all times. All equipment must be OSHA certified and/or meet all OSHA requirements. B. Supplies: The Virginia Department of Health or the City of Newport Newsshall provide light bulbs, and fluorescent tubes. All other cleaning and housekeeping stock supplies shall be furnished by the Contractor. All supplies furnished by the Contractor shall be made available for inspection and approval for use by the Contract Administrator. Sufficient supplies to prevent out-of-stock situations shall be always maintained on the job site and made available to the Contractor's employees for use in performance of required services. Contractor shall maintain a supply log for ordering purposes and utilization monitoring. 5. The Contractor shall provide a copy of the Scope of Work to all personnel assigned to provide the services in order to assure quality performance. Each personnel assigned will sign a statement of understanding of scope of the services rendered for this facility. 6. The Department and Contractor may mutually agree to modify the time(s) indicated in the cleaning schedule. 7. Upon cancellation or termination of the contract, the Contractor shall return any and all access badges and keys, including any copies made by the Contractor, to the Contract Administrator no later than 24-hours after cancellation or termination. Should the Contractor not return access badges and all keys within 24 hours, the Contractor shall be responsible for all associated costs of replacing the locks and keys for the facility.
