Host an outreach and engagement event for up to 500 people to promote immunizations, school physicals, WIC, safe sleep, car seat, and home visitation programs. Share outreach and engagement numbers (approx. Number of people engaged) with RCAHD within 60 days of the event. Host a training for up to 85 members of the local MCH workforce on trauma informed maternal child health support with a focus on post partum support, including breastfeeding. Share number of providers/partners educated and associated outcomes with RCAHD within 60 days of the event Pull, analyze, and share hone visiting program data regarding the post-partum visit with RCAHD. Work with RCAHD to identify data about hone visitation clients and their post-partum visits, along with various demographics and risk factors, such as age, race, income, geography, and substance use or mental health disorder, number of children, etc.... Pull and share data analysis quarterly Participate in RCAHD-led maternal child health meetings, including Home Visiting Partner calls and new maternal child health perinatal partner meetings. The Department will: Assign a Contract Administrator to oversee the contract. The Contract Administrator is responsible for making sure that the services are provided in accordance with the terms of the contract and cannot make any changes to the contract. Any changes to this contract shall be in accordance Section VII., Paragraph E., of this contract and will come from VDH’s Office of Purchases and General Service (OPGS).
