Minimum Services Required: 1. Advanced primary Care 2. Treatment of chronic and acute episodic injuries, illnesses, and accidents i. Examples include: Upper respiratory infections, Strep throat, Bladder infections, Ear infections, Sinus infections, etc. 3. Comprehensive physical examinations 4. Illness prevention and risk-reduction counseling and support i. Examples include: Smoking cessation, Weight management, Diabetes management, Injury prevention, etc. 5. Immunizations i. Hepatitis B&C, Tetanus, Influenza/Pneumonia, etc. 6. Laboratory Services 7. Provide monthly utilization reports as required by FCPS and their benefits consultant 8. Targeted and proactive outreach to members identified with chronic illnesses and highrisk individuals 9. Nutrition Counseling 10. Conduct annual biometric screenings 11. Develop and manage wellness program to include incentives 12. Participate in annual benefits fairs, when held 13. Participate in Summer new hire orientation 14. Conduct TB screening, pre-employment physicals, and tests for all new hires 15. Occupational Health Services TBD but may include: i. Completion of tests to aid city in OSHA Compliance Standards ii. Random Drug Testing, DOT and Non-DOT iii. DOT Certification and Re-Certification iv. Hearing Testing v. Visual Acuity vi. Spirometry vii. Breathe Alcohol Testing viii.Dispensing of Occ. Med Medications ix. Referrals
