Specifications include, but are not limited to: The Contractor shall furnish preventative maintenance service for HVAC related equipment and supplies (including all necessary parts, labor, supervision, materials, equipment, and permits, if applicable) and "on-call" repair services on all HVAC equipment. Preventive maintenance shall include all adjustments and servicing required to maintain systems and equipment in good operating condition, in accordance with the manufacturer's specifications and Attachment F, titled "General Equipment Preventive Maintenance Guidelines". The Contractor shall deliver to the Cemetery Superintendent all copies of warranty certificates, maintenance manuals, and other pertinent data as it relates to replaced parts and equipment. Travel charges of one (1) hour to the job site will be allowed for service call or preventive maintenance. Travel shall be paid in increments of fifteen (15) minutes. If Contractor needs to return to the location to repair or PM a location due to the fault of the Contractor, then travel shall not be paid. Materials shall be provided at the Contractor’s actual invoiced cost. Cost plus charges is not allowed. A copy of the actual invoice must be provided or if the truck stock indicates that on the invoice. B. Preventive Maintenance Services: 1. The Contractor shall be prepared to schedule an initial preventive maintenance service visit within ten (10) days of the award of the contract. The Contractor shall use the maintenance guidelines outlined in Attachment F as the starting point for preventive maintenance. Seasonal changeover service shall be performed during the requested preventative maintenance visits. These visits are requested to occur twice per year, once in the late spring and once in the late fall to prepare for the upcoming summer and winter seasons. 2. Upon completion of the initial service of the HVAC equipment, the Contractor shall submit to the cemetery a written report, a form of which is Attachment G. This report shall identify the date(s) service was performed, and the following additional information on each piece of HVAC equipment: a. The manufacturer, model, and serial number. b. A detailed, narrative statement as to the condition of the equipment, indicating work that apparently has been performed previously, and an estimate of the age. c. Specific recommendations for any major repair or modification, which will enhance the operation of this equipment. d. An estimate of the cost to accomplish the work recommended in item c above. e. A list of service items, to include: filter type, size, and number; drive belt size, type, and number; any other information related to servicing the equipment including the recommended service interval (quarterly, semi or annual basis).
