A. The Contractor shall provide all labor, supervision, vehicles, equipment, tools, parts, materials, water, debris disposal, confined space equipment and certified confined space personnel to furnish and install a missing concrete wall inside the existing drop inlet located across from 7806 Hampton Park Boulevard in Chesterfield County. See Attachment F for map of location and photo of existing drop inlet. B. The Contractor shall provide all employees with necessary safety equipment required to meet OSHA and VOSH standards. C. The Contractor shall provide materials conforming to the material standards contained in the 2020 VDOT Road and Bridge Specifications or latest edition, Section 217. Concrete and flowable fill shall be provided from a VDOT approved plant. See Attachment G. Concrete shall be A3, or as directed by the VDOT Contract Administrator. D. All work performed under this contract shall be accomplished in accordance with the 2016 or latest version of the VDOT Road and Bridge Standards, Sections, 100 and 200, VDOT 2020 Road and Bridge Specifications or latest edition, Sections, 502 to 504, and the latest edition and any revision of the Virginia Work Area Protection Manual (VWAPM). The Contractor’s equipment shall meet Occupational Safety & Health Administration (OSHA) and Virginia Occupational Safety & Health (VOSH) standards. Additionally, the Contractor shall remain in compliance with all OSHA and VOSH personnel, equipment, and safety regulations. E. Prior to digging, the Contractor shall contact “Miss Utility of Virginia” for marking underground utilities prior to the commencement of work. The Contractor shall not dig until verifying the marking has occurred. The Contractor shall be responsible for any damages caused when “Miss Utility of Virginia” guidelines are not followed. F. All equipment used for this work is subject to VDOT inspection prior to being placed into operation. All equipment shall be suitable for the intended purpose and the type of work being performed. G. The Contractor shall perform the repair in a neat, systematic and workmanlike manner. H. The missing concrete wall inside the drop inlet (DI) shall be replaced using the following methods: 1. Contractor shall provide and use a combination truck to hydro-excavate the area of the sink hole next to the DI to expose the void in the top wall of the DI. The Contractor shall be responsible for securing a suitable disposal location for all waste/debris materials and any costs associated with disposal. 2. Contractor shall provide and use a combination truck to vacuum and jet inside the DI to remove existing gravel and mud. The Contractor shall be responsible for disposal location and costs for gravel and mud, etc. See Section N. Disposal. 3. Contractor shall furnish and set up sandbags inside the pipe to help contain mud and debris, to prevent downstream impacts to adjacent environmental resources. Contractor shall remove and dispose of sandbags when finished using them. 4. Contractor shall furnish and use Confined Space equipment with certified 3-man crew to enter the DI to build a form in-place with wire for rebar to pour new concrete for void. 5. Contractor shall furnish and pour concrete from outside the DI using the earth for sidewalls of the form. 6. Once the concrete is cured (up to 48 hours later), Contractor shall return with the Confined Space equipment and personnel to enter the DI and remove and dispose of the form. The form shall be removed no more than 72 hours after installation. The Contractor shall notify the Contract Administrator in writing within 2 hours of form installation and removal. 7. Contractor shall furnish dirt and backfill the sinkhole to existing grade. Backfill work shall be performed in accordance with the 2016 VDOT Road and Bridge Standards, Section 303.03(b) 8. Contractor may remove and dispose of up to three (3) sections of existing sidewalk, if necessary. VDOT will replace any of the 3 sections, if removed. Contractor shall be responsible for disposal location and costs if removed. See Section N. Disposal. 9. The Contractor shall take every precaution for the protection of people who may come on the work site or be affected by the Contractor's operation and ensure all work/job areas are maintained clean and free from safety hazards. 10. The Contractor shall take every precaution to continuously maintain adequate protection of all their work from damage and shall protect VDOT property from injury or loss arising in connection with this Contract. The Contractor shall adequately protect adjacent property to prevent any damage to it or loss of use and enjoyment by its owners.
