Specifications include, but are not limited to: 1. Provide services as detailed in 2 through 12 below, for the facility located at Bon Air Juvenile Correction Center. The provider shall submit an accounting of hours worked each month to the facility’s Health Authority. The Provider will be paid for only the hours worked up to the required total. Services provided in less than full hour increments shall be invoiced and paid in one-tenth of an hour increments. Lunch breaks shall be unpaid and time taken must be deducted from total monthly hours. If hours per month reach 16 and additional hours of service are required, the specific number of additional hours must be approved in writing by the facility’s Health Authority before services are provided or the Contractor will not be paid for said hours. If service hours exceed the per month hours, any additional hours of service shall be approved in advance by the facility’s Health Authority and shall be paid at the awarded hourly rate. Proposals will be considered that bill by the patient. 2. Provide all equipment needed to conduct exams. 3. Provide general optometry services to include performing eye examinations, prescribing prescription eyeglasses, and administering medications, provide interpapillary distance (IPD) and frame size, and other related duties. 4. Record a summary of the problems and/or symptoms, the diagnosis or diagnostic hypotheses, treatments, medications, or any other appropriate information in each patient’s medical chart at the conclusion of every consultation. 5. Adhere to the Formulary established by the Department of Juvenile Justice. 6. Consult with the Department’s Office of Health Services regarding care of residents when treatment questions arise. 7. Immediately notify the Department if license is suspended or revoked; if any malpractice claims are filed; if any professional disciplinary action is taken; or if he/she has any physical, mental or emotional problems, which might impact performance of assigned duties. 8. Make referrals through nursing department to specialists when necessary. Utilize designated consult form for all referrals. 9. Use on-site office space provided by the facility during scheduled visits. 10. Provide portable optometry equipment for use at the facility...
