A. Provide free primary and specialty care services to patients. B. Provide access to free laboratory and imaging services. C. Provide free medications. D. Provide free care coordination. E. Provide free case management. F. Provide collaborative care and medication for general/situational behavioral health needs. G. Provide referrals to Loudoun County Department of Mental Health, Substance Abuse and Development Services (MHSADS) for individuals with a diagnosis of serious mental illness (SMI). H. Provide universal substance abuse screening and referrals to MHSADS. I. Treat sick patients, both new and existing, within two (2) business days of being contacted. J. Provide referrals to a comprehensive medical home for eligible patients. K. Track the following Performance Metrics. 1. Number/percent of patients receiving an evening appointment with a two-week period. 2. Number/percent of patients receiving a flu vaccine. 3. Number/percent of no-shows for appointments. 4. Number of referrals to comprehensive medical home. 5. Number/percent of sick patients seen within two (2) business days of being contacted. 6. Number of patients receiving referrals for behavioral health needs.
