All proposals must be made on the basis of, either meet or exceed, the requirements contained herein. All offerors must submit a proposal demonstrating their capability for rendering all of the following prescribed tasks integral to reduced fee comprehensive medical home services as described in this Section 5.0: 5.1 Reduced Fee Comprehensive Adult Dental Services Provide access to comprehensive oral health care services to new or existing low-income and uninsured Loudoun County adult patients A. Preventive Dental Examinations 1. X-rays and oral cancer screenings 2. Routine cleanings B. Acute Oral Health Services, to include: 1. Restorative (fillings) 2. Endodontics (root canal therapy) 3. Oral Surgery 4. Hard and Soft Tissue Biopsies 5. Periodontics 6. Prosthetic Appliances and Services 7. Other Emergency Interventions as needed C. Intake services 1. Verify participants meet eligibility qualifications to include: a) Resident of Loudoun County; b) Age 18 or older; and c) Annual income at / or below 200% of Federal Poverty Guidelines. D. Track the following Performance Metrics: 1. Number / percent of patients receiving detailed treatment plan outlining all recommended treatment. 2. Number / percent of comprehensive dental examinations (including radiographs as ordered) to determine the current state of oral health for Loudoun County patients approved and initiated into the program. 3. Number / percent of Loudoun County patients with an improved state of oral health resulting from services. 4. Number / percent of Loudoun County patients requiring emergency care resulting in resolution of the emergency dental condition.
