The Town of Smithfield Department of Public Works & Utilities is requesting proposals to provide all labor, equipment, material, transportation, permits, and fees to install Curbing, sidewalk, driveways and miscellaneous concrete as directed. All work shall be completed in a first-class workmanlike manner. The town may reject all or any portion of the work that is not in accordance with the approved specifications or is deemed to be unsatisfactory. The town shall withhold payment for all work until all unsatisfactory work has been corrected. The scope of work consists of the following: • Concrete as specified by the Town will be provided by the contractor. • The contractor shall be responsible for disposal of materials. • The installation of all concrete shall be in compliance with the Town of Smithfield design standards and the Hampton Roads regional standards. • Provide hourly rates for labor and equipment associated with this proposal • The contractor shall be capable of mobilizing within (10) working days after notification. • The contractor shall maintain vehicular and pedestrian traffic at all times. One way traffic will be allowed with the use of trained flaggers and prior approval from the town. The contractor shall maintain traffic for both private and commercial entrances at all times. The contractor shall follow guidelines set for in the MUTCD – work zone safety, and the latest edition of the VWAPM. • The contractor shall provide equipment, material, operators and laborers for any required work. • Work hours shall be 8am-4pm unless otherwise approved by the Town. • It shall be the responsibility of the contractor to remove and dispose of surplus concrete in an appropriate manner. • The contractor shall be required to install curb and gutter as specified by the town. All curb shall get a 6” base of 21A stone. • The contractor shall be required to install sidewalk that is no less than 4” thick. • The contractor shall be required to install driveways no less than 4” thick. • The contractor shall be required to install driveway aprons no less than 7” thick • The contractor shall be required miscellaneous concrete as directed by the town. • The contractor shall abide by all safety guidelines involving OSHA and shall adhere to the Town of Smithfield Safety Program. • The contractor shall maintain records of all labor and materials and submit them to the town prior to payment.
