Specifications include, but are not limited to: A. The Contractor shall provide specialized medical, psychological, psychosocial, and educational services in accordance with the individual patient’s needs. Other evaluation services may include speech/audiology, and evaluation for occupational and/or physical therapy. These services shall be provided to children and young adults under the age of twenty-one (21) residing within the Contractor’s catchment area who are suspected to have or have been diagnosed with developmental, emotional or behavioral disorders or who have multiple risk factors for those conditions. The Contractor’s catchment area includes the following counties/cities: counties of Accomack, Isle of White, Northampton, Southampton, and York; and cities of Chesapeake, Franklin, Hampton, Newport News, Norfolk, Poquoson, Portsmouth, Suffolk, and Virginia Beach. The Contractor will also be expected to assure that services are equitably available to children, including those who are under-insured or uninsured. These services shall include an initial interdisciplinary individualized evaluation (which may include any or all of the following, depending on the child’s presenting complaint: a pediatric medical examination, nursing assessment, psychosocial assessment, psychological assessment, educational evaluation and speech//language/audiological evaluation) for the purpose of identifying previously undiagnosed developmental, emotional, or behavioral disorders and connecting these children and families with appropriate resources. As a result of this goal, services will also include initial referral to appropriate resources for follow up care and treatment planning. B. The Contractor shall appoint one of its full-time employees as “Director” of the program activities of the CDC whose responsibilities include monitoring of the services provided and supervision of the day-to-day operations. Further, the “Director” shall be available as a key contact for VDH and participate in meetings as requested by VDH. Meetings are anticipated to be at a maximum of four (4) per year. C. Services offered (as identified in this section) at each clinic location may vary according to the needs of the community, expertise of professional staff and the overall goals and objectives of the current program. The Contractor shall provide the scope of service under the following provisions: 1. Services limited to one time individualized evaluation; 2. Services tailored to each child’s presenting complaint and necessary to complete the individualized comprehensive evaluation; 3. Referral to appropriate follow-up resources; short term follow-up coordination. 4. Services meeting the linguistic needs of the child and family in accordance with Federal Limited English Proficiency standards, including appropriate translation and interpretation services; 5. Services provided in a culturally competent manner; and 6. Engagement of the family in all phases of diagnosis.
