A. DFS will send equipment and weights listed in Attachment A to the Contractor to perform off-site calibration services as specified in the following mandatory requirements. B. For each off-site calibration, the Contractor shall: a. Utilize NIST traceable standards for the calibration. b. Affix a sticker to each piece of equipment or container indicating the date of calibration and the date recalibration is due (one year following at the end of the month). c. Provide a Calibration Certificate for each piece of equipment or weight meeting the criteria specified in ISO/IEC 17025, which contains the following: i. Report accurately, clearly, unambiguously and objectively; including all information requested by the customer and necessary for the interpretation of the results and all information required by the method used; ii. Title; iii. Laboratory name and address; location where test was carried out if different from lab; iv. Unique identification number; identification number on each page; clearly identify document end; v. Customer name and contact information; vi. Identification of method used; vii. Description of and unambiguous identification of, the item(s) being calibrated; viii. Date of receipt of the test or calibration item(s), and the date of sampling, where this is critical to the validity and application of the results; ix. Date(s) of performance of the laboratory activity; x. Date of issue of the report; xi. Reference to and description of sampling plan and sampling method used where relevant to validity or application of results; xii. Statement to the effect that the results related only to the items calibrated; xiii. Test results with units of measurement; xiv. Additions to, deviations, or exclusion from the method;
