To establish a contract with a professional Consultant/Physician that will provide physician examination and evaluation services. County staff will contact provider to schedule employee appointments. Provider must have capability to see employee within 14-21 calendar days of initial contact. County staff will be responsible for informing the employee of the appointment. Prior to the employee’s appointment, County staff will send the provider medical reports from the employee’s physician(s) along with current job description via secure site upload (or fax, if secure upload is not available). Prior to the employee’s appointment, the provider will be responsible for sending the employee any paperwork and/or questionnaires needed. Physician Services/ Types of Physicals/Tests: • Annual Bus Drive Physicals o Includes the exam, the in-house u/a, visual acuity, whisper hearing • New Hire Physicals o Includes the exam, the in-house u/a, visual acuity, whisper hearing o Optional additional services to be added (TB Skin Test or / Urine Drug Screen testing) • TB Test for New • TB Risk Assessment (Annual physicals only) • Urine Test - an in-house urinalysis is included in the Annual and New Hire Bus Driver Physicals; Add on individually if desired.
