The purpose of this Request For Proposals (RFP) is to solicit for a homeless service provider (“Partner”) to provide intensive case management and wrap around services to assist in the implementation of a pilot Project to provide emergency shelter and rapidly rehouse homeless youth (the project Participants). It is anticipated that the number of project Participants may number between 10 and 30 individuals. The Partner organization will be an integral part of the project team and will be the primary point of contact for project Participants. The services will include the provision of emergency shelter and rapid rehousing assistance along with intensive case management and wrap around services. Referrals will be initiated by area School Divisions, area Social Services Departments, or the York County Housing Division.
