A. The Contractor shall provide audiology assessments, management of hearing assistive devices and aural re-habilitation services to students from the VSDB. B. The Contractor shall perform administrative functions for establishment and maintenance of a quality program. C. The Contractor shall coordinate, cooperate and consult with agency staff and related outside agencies and organizations for the purpose of integrating the audiology program with other school therapy programs and goals. D. Direct audiology services shall be conducted by a licensed Audiologist through the Virginia Board of Audiology and Speech Language Pathology. The same individual must provide services to the same students on a regular basis. E. The Contractor shall schedule students for audiology services while they are on campus participating in school programs. Non-residential students shall receive audiology services during school hours. All scheduled sessions will be coordinated with VSDB staff. F. The Contractor shall provide evidence of medical/malpractice insurance to VSDB. G. The Contractor shall provide hearing screenings/assessments to employees relating to the screening program. H. Special Conditions: 1. The Contractor shall provide VSDB with the following reports by the specified due dates: a. Initial, pre-program evaluation on referred students. b. Daily progress notes on each student receiving services. c. Quarterly reports for inclusion in the student report cards. d. Recommendation for continued services with specified goals and objectives for inclusion in the IEP 10 days prior to the scheduled meeting. e. Reports of auditory status including Functional Listening Assessments, as determined by students’ IEP committee, for triennial/eligibility meetings. f. Contract must provide all documentation for Medicaid billing. 2. Upon VSDB referral of a new student whose IEP identifies the need for audiology services including assessment and/or aural re/habilitation services, the contractor will provide initial contact toward the student evaluation within five work days. 3. Contractor shall receive written authorization from VSDB before any arrangements are made for additional services or for any equipment purchase. 4. The Contractor shall participate in IEP and triennial meetings or call parents to explain services and recommendations when requested by VSDB staff. 5. The Contractor shall have experience with the provision of pediatric audiological testing, (audiometric thresholds, speech audiometry, impedance testing, otoscopy, optoacoustic emissions, evoked potentials), including Functional Listening Assessments, amplification management and aural re/habilitation services to children with sensory disabilities to include Deafness/Blindness/Deaf- Blindness, preferably in a school setting. The Contractor shall be willing to learn, if necessary, how to write IEP goals and objectives as well as other education related documentation. 6. Shall cooperate with VSDB in fingerprinting, criminal history and child protective services investigations per Code of Virginia 63.1-248.7:2. 7. Shall coordinate services through the VSDB Principal. 8. Shall work to arrange for service delivery in scheduled blocks of time to accommodate VSDB class scheduling. 9. Shall bill for services provided on a fee for service basis. Billing must reflect direct and indirect services with respective cost structures. Direct services shall be student sessions/walking to and from classroom if student is under your supervision/IEP meetings or consults/teacher consults. Indirect services shall be training/all paperwork (including paperwork related to IEP’s and Medicaid, cleaning up and setup). No payment will be made for time from port-to-port or travel to and from services on campus. Time spent in billings and in administrative meetings concerning contract issues are to be in Contractor’s overhead and not billed to VSDB. 10. Missed student appointments will be documented and a monthly report on all will be sent to the Principal. The audiologist will use “cancellations” or “no show” time to fulfill contractual obligations. Priority will be given to providing direct audiology services to available students that can be substituted during the time frame. If no direct service can be arranged, then the therapist may utilize time to complete other contractual obligations, i.e. documentation, consultations, etc. Contract administrator may approve payment on student missed appointments...
