Specifications include, but are not limited to: 1. Service Description Contractor shall provide medical assessments, case consultation and psychiatric services for Woodside. Contractor shall meet regularly with the Woodside Director to evaluate services that are being provided and determine a schedule of services to be provided in the coming month(s). Services to be provided by the Contractor include: a. Physical assessments and other medical assessments; b. Psychiatric assessments; c. Medication management and review of medical histories; d. Consultation/training to staff as appropriate; e. Face to face client contacts; f. Provide nursing staff that are approved and trained as Sexual Assault Nurse Examiner (SANE); g. Allow for Hope Works counselors to be available upon request; h. Provide client reports; and i. Evaluation at UVM Medical Center for acute medical or psychiatric care. 2. Target Population and Eligibility Criteria a. Woodside serves youth from age 10 through 17, and some youth over the age of 18 who choose to remain voluntarily, who require treatment in a secure setting, when less restrictive settings are not adequate. These children and their families will be recipients of these services. b. Woodside staff, State social workers and courts will receive training and consultation as appropriate. c. Contractor shall inform any child in State custody or any individuals in an open family case who are receiving services under this contract, that information gathered during the course of providing services to them as well as information about services may be disclosed by the Contractor in closed Juvenile court proceedings upon service of a subpoena. 3. Eligibility Criteria Youth and their families admitted to Woodside by a court order or delinquents placed by the State’s Department for Children and Families Administrative Process. 4. Geographical Area Woodside serves youth across the State of Vermont at the Colchester, Vermont location. 5. Purpose The program is intended to provide intensive psychological, behavioral, educational and substance abuse services to address immediate and long-term mental health and behavioral treatment and change.
