Description

The UFCI will start providing firefighter screenings in August 2025. Screenings will continue throughout the year with approximately 500 screenings performed each year.
Firefighters will come to vendor locations to receive MRI and/or CT Scan services. The vendor will submit invoices to the UFCI on a regular basis for services performed. Firefighters will not pay for services.
Pricing is to include delivery of test results.
Additional services can be added within the nature and intent of the scope of work, if cost is determined to be fair and reasonable, by amendment signed by both parties.

Please complete and attach the vendor response form (attachment A), add any additonal requested or reference documents by the dates and times listed for consideration.

The Attachment B- Independent Contractor Agreement (ICA) draft is provided for reference and only needs to be returned if a vendor is providing redlines for the Universities consideration during contracting. If the agreements terms, along with the RFPs is acceptable the resulting contract will consist of a simple cover page, the RFP, and the ICA with final appendices.
