Specifications include, but are not limited to: CONTRACTOR will ensure ultrasounds are performed by qualified staff in accordance with industry standards and as defined in the contract purpose. Quality of service and performance will be measured by CONTRACTOR’S ability to respond within agreed upon time frames, reporting, record-keeping, invoicing in accordance with contract requirements, and by measuring and evaluating evidence provided by the CONTRACTOR and monitor of the CONTRACTOR’S compliance with each and every term and condition as defined in Attachments A through C. DETAILED SCOPE OF PERFORMANCE CONTRACTOR shall perform ultrasound exams on an as needed basis, usually Monday through Friday during normal business hours, to include: Abdominal Ultrasound; Breast Ultrasound; Cardiac Carotid Artery Profile Ultrasound; Doppler Ultrasound; Extremity Non-Vascular Ultrasound; Pelvic Ultrasound, including Transvaginal Ultrasound; OB Ultrasound; Renal Ultrasound; Testicular/Scrotal Ultrasound; and Other Ultrasounds as needed. Most tests will be conducted on site at the Utah State Prison, in Draper and some tests may be conducted at the Central Utah Correctional Facility, in Gunnison. CONTRACTOR will provide written reports from a licensed certified Radiologist on all test conducted within seven days. Reports must be conducted by a licensed company of certified radiologists that meets the requirement of the State to read ultrasounds. Reports must include the name of the subcontractor interpreting the test and the CONTRACTOR’s name on their letter head. The CONTRACTOR is solely responsible for the cost to provide any and all reports. CONTRACTOR must immediately report any emergency or acute findings. CONTRACTOR will provide all of the necessary supplies and equipment. CONTRACTOR shall meet all applicable industry standards, comply with all applicable UDC policies and procedures, and joint policies or protocols developed between the CONTRACTOR and UDC.
