Specifications include, but are not limited to: 1. Provide experienced licensed staff, outpatient facilities, equipment and locations to serve children who have a non-progressive neurological condition to include cerebral palsy and spina bifida within Utah. 2. Identify the number of anticipated children to be served. 3. Provide licensed physical therapists, occupational therapists and speech therapists that have experience with pediatrics with neurological conditions. 4. Staffing plan for serving the number of anticipated participants of the Fund. 5. Implement a recruitment process to identify and enroll participants. 6. Provide intake assessment of clients to identify if they qualify for the fund. To qualify: a. Have a neurological condition that is not a traumatic brain injury or spinal cord injury and is non-progressive. b. Client must be a resident of Utah. c. Have no other means to pay for services. d. A child between 3 and 22 years of age 7. Contractor must have 70% of qualified individuals show progress in all three categories of the International Classification of Function (ICF) model: a. Body Structure/Function b. Activity c. Participation
