Specifications include, but are not limited to: A BIDDER must be licensed in the State of Utah with no disciplinary actions. B. Sewer grinder services to be performed every six months, services to include but not limited to: 1. Perform grinder inspection 2. Inspect and tighten the cutter stack. 3. Inspect the grinder drive and grease gear reducer. 4. Inspect and test motor amp draw, voltage leg to leg and temperature. 5. Provide the amp draw, voltage and temperature on the work order/ticker that will be provide to NUCC at the completion of the service. 6. Supply appropriate maintenance on the grinder to prevent malfunction. C. Sewer pumps (2) services to be performed at the same time as the grinder, services to include but not limited to: 1. Grease the rails guiding the pump up and down of both pumps. 2. Pull up the pump sensors and make sure it is cleaned off and return it gently back into the pit. 3. Check amp draw and voltage leg to leg on both pumps make sure it is in an acceptable range, to help identify future failures. 4. Provide the amp draw measures on the work order/ ticket that will be provided to NUCCC at the completion of service. 5. BIDDER shall provide emergency pumping services, including the use of a pump truck if debris can visually be seen that would harm the pumps. D. Emergency hourly rate for non-working grinder or pumps. Any emergency repair for non-working sewer grinder or sewer pumps will require quotes from the BIDDER for repairs. Per-set emergency hourly rate per man hour plus materials if needed, to fix non-working pumps or grinder or if needed remove and replace with new equipment. If Corrections, has the equipment available, BIDDER will use that first, if not BIDDER will purchase the new equipment and install it. Corrections will then reimburse BIDDER for cost. The quote will be provided to the Contract Monitor who will request the Center Director or authorized designee for approval. Services shall not begin until the Center Director or designee has signed the quote for the repairs. UDC will not be financially responsible for any services completed without the Center Director or designee signature. The signed quote will be attached to the invoice for payment. E. Plumbing services 1. Respond to plumbing request within 24 hours of request. Emergency request within 4 hours. 2. Perform auger service to unclog drains 3. Provide quote for jetter service if clog cannot be unplugged by auger. Center Director or authorized designee must sign the quote giving permission to use the jetter before the service can be completed. A copy of the quote must be provided with the invoice for payment. 4. Provide UDC with a Service Ticket for service provided and signed by Contract Monitor or designee. 5. Provide services Monday – Friday, 8:00 a.m. to 5:00 p.m. (regular hours) 6. Provide emergency service to include: after hours, 5:00 p.m., Saturday and Sunday, and any State and Federal Holidays (New Years Day, Martin Luther King Day, Presidents Day, Memorial Day, July 4, July 24, Labor Day, Columbus Day, Veterans Day, Thanksgiving, Christmas).
