Specifications include, but are not limited to: Service Requirements: The Contractor shall: A. Provide counselors licensed under Utah Administrative Code R501-21 and R156-60a to help connect each eligible Individual with state services, certified peer support and advocacy, and home visitation services. The Counselor will administer an assessment and determine eligibility based on the assessment and the ASAM criteria B. Provide a certified peer support program. Peer support specialists shall have received training from an accredited program and apply for certification through DHHS Office of Substance Use and Mental Health Certified Peer Support Specialist Program. C. Provide mental health supports to participating Individuals through home visitations from appropriate licensed counselors. This will include assessments, personalized care plans, counseling services, and health education. D. Expand screening, brief intervention, and referral to treatment services to the target population. E. Perform in-home health education and outreach activities designed to engage Individuals. F. Expand comprehensive evidence-based treatment and recovery services, including medication for OUD for Individuals. G. Provide comprehensive wrap-around services to Individuals, including support in finding stable housing, transportation, job placement, training, and childcare. H. Facilitate community resource referrals, care coordination, and care management between patients, community outreach teams, and healthcare providers for Individuals and their households. I. Continue to provide and update the Individual’s current ASAM criteria treatment plan. Contractor will ensure that the appropriate level of services is provided as determined in the Individuals current ASAM criteria recommended level of service. J. Provide preventative physical and mental health services to decrease risk of recurrent opioid use and hepatitis and human immunodeficiency virus exposure. K. Provide indicated and evidence-based services for mental health concerns, including support to reduce the symptoms of postpartum depression and anxiety. L. Expand comprehensive evidence-based medical monitoring and recovery supports for babies with NAS.
