All correspondence must be made through the Vendor Portal. Specifications include, but are not limited to: The contractor will assess the populations they already serve in whatever capacity they do so: feeding, social welfare support, counseling, sheltering, legal support, etc. The contractor will conduct monthly assessments, per event, of the populations to which the Contractor already provides direct services; e.g. feeding, social welfare support, counseling, sheltering, legal support, etc. to determine: a) Number of persons vaccinated b) Which of the available vaccinations was received c) Completion of vaccination series; dose(s) received / booster d) Reasons for non-vaccination, the assessors need to determine the status of each person queried i. Barriers to access; e.g., no transportation, limited appointment dates or office hours, limited education about sites to obtain vaccination ii. Hesitancy iii. No access to vaccine information iv. Language barrier v. Homeless vi. Undocumented vii. Mental challenges viii. Other (any reason not listed)
