Specifications include, but are not limited to: Coordinate and track veterans services at the community level with the support of a veteran service coordination and referral system to be used by all participating veterans service providers; promote and establish a team approach among community service providers to facilitate resource coordination and information sharing among network participants and their partners using a veteran service coordination and referral system that compiles and reflects all services available and referrals made for individual veterans referred through the network; and, organize network activities, including providing training to front-line staff of veterans service providers on the use of the veteran service coordination and referral system to maintain the fidelity of the program. Veterans Coordinators shall be employed by a successful Applicant in the regional coordination centers as full-time employees, and their job duties and/or responsibilities will include, but are not limited to: providing guidance to the network of organizations to facilitate integrated and streamlined services for veterans; maintaining and managing a shared veteran service coordination and referral system to be used by all participating network organizations to coordinate shared service referral information; training front-line staff in each of the participating network organizations on the use and reporting features of a shared veteran service coordination and referral system; serving as an educational resource to network organizations and local entities interested in working with the established network to support veterans by offering peer-to-peer and employer workshops or serving as panel guests to discuss the benefits of connecting through the veterans network; developing Standard Operating Procedures (SOPs) for how collected information may be accessed, and by whom, to support veterans’ access to services regardless of location, and how to retain veterans’ referral records securely for a minimum of three (3) years; implementing a plan to gather, track, and secure veterans’ referral information across multiple organizations using a shared veteran service coordination and referral system; building and strengthening collaborative relationships among organizations serving veterans and local service partners; hosting events among participating network organizations to share information on policy and procedural impediments, service delivery, funding details and/or opportunities, and other topics that may improve opportunities for veterans seeking services within their network
