Specifications include, but are not limited to: To provide skilled nursing facility care to University Health System patients beginning November 1st 2019,who are uninsured, have CareLink or Medicaid, and who are medically ready to transition to the lower level of medical care provided at skilled nursing facilities, rather than remaining in an acute setting. Skilled services will be all-inclusive and billed at a daily rate to include but not limited to Physical Therapy (PT) Occupational Therapy (OT) and Speech Therapy (ST); Wound Care/supplies, Semi-private room, IV medication administration; Pharmaceuticals; and Dietary services including tube feedings. The daily rate will specify frequency of Therapy Services, as well as those services that may be excluded, requiring an additional fee. Additionally, provide a second daily rate for those patients requiring less-intensive skilled care (i.e.: IV antibiotic administration without PT/OT/ST). Short-term skilled care will be provided with the expectation of transitioning the patient back to their home community with or without home care services and community resources
