Mexia State Supported Living Center (MSSLC) requires the services of a Podiatrist (Contractor) to see and treat individual residents on the MSSLC campus. At the request of the MSSLC Department Physician, the Contractor will treat foot diseases. The Podiatrist (Contractor’s) office must be located no more than 100 miles of Mexia State Supported Living Center, located at 540 Chapel Drive, Mexia, Texas 76667-3490. Contractor will provide: a) Examinations, evaluations, diagnosis, and specific foot treatment if required; b) On campus clinic two to four times monthly depending on individual needs, Monday-Friday between 8:00 am – 5:00 pm, to be scheduled by HHS personnel; c) Submit clinic service dates to HHS Medical Director; d) Maintain current Podiatrist (Doctor of Podiatric Medicine) license with the Texas Department of Licensing and Regulation; e) Maintain current liability insurance coverage sufficient to cover the Contractor’s activities; f) Submit itemized invoices to include: name, address, and telephone number of the Contractor; contract purchase order number; identification of services provided; dates services were delivered; total invoice amount; Contractor’s Texas Identification Number (TIN); and any additional payment information that is required by the Contract. g) Individuals living at Mexia State Supported Living Center will have the best possible foot care through early detection of problems and treatment. h) An average of 80 individuals will be treated monthly, 20 per week. Numbers are contingent upon Physician’s consults. i) Ninety percent of individuals needing treatment will be seen monthly and will be under the Podiatrist’s observation until improvement has been noted.
