Specifications include, but are not limited to: 1. Except as stated otherwise, transportation and delivery of all equipment, supplies, and manpower necessary to complete the Medical Waste Disposal Services shall be the responsibility of the Contractor. TTUHSC will not accept any additional charges for supplies such as sharps containers, or shipping or handling fees. Contractor must provide necessary leak proof containers with lids or other acceptable closure and a plastic bag liner per container. 2. Contractor will provide bar code labels for each pick-up location (site) containing, as a minimum; Generator Name, Address, and Phone number, and the Transporter Name, Address, and Phone number. Contractor will provide additional containers, liners, required labeling, and bar coding for the disposal of expired or contaminated nonhazardous pharmaceuticals, pathological, and trace chemotherapy wastes. 3. Contractor will provide a single monthly billing to TTUHSC Safety Services office for all serviced locations. The invoices must address, for each site, the number of containers, weight of the waste (in lbs.), and type of waste disposed, and total cost. Send invoices to TTUHSC Safety Services Department, Room BA120, 3601 4th Street, Mail STOP 9020, Lubbock, Texas 79430-9020. 4. All Contractor employees handling any TTUHSC medical waste shall be trained in emergency spill response actions. The Contractor shall have available at all times, when. handling and transporting TTUHSC’s medical waste, sufficient spill control equipment and supplies to allow for the control and containment of a worst-case scenario involving the types and amounts of wastes being handled and transported. 5. Contractor must furnish a “shipping manifest” for each shipment of waste, immediately after the shipment is picked-up. Contractor must be fully insured and provide proof of insurance upon request. Contractor must provide current copies of operating and disposal permits upon request. All transportation and disposition of the special/medical waste must be in accordance with applicable current State of Texas and Federal Regulations. If any subcontractors are to be used, all of the above listed documents must be submitted for each subcontractor.
