The purpose of Amendment 4 is to:

1.  Correct the ISBN Numbers for the following CLINS: (Details in Attachment 7 - Amendment 4)
     a. CLINS 0005 and 0006 - Principles & Techniques of Patient Care, 6th Ed
     b. CLINS 0009 and 0010 - Orthopedic Physical Assessment, 7th Ed.
     c. CLINS 0011 and 0012 - Fundamental Orthopedic Management, 5th Ed.
     d. CLINS 0023 and 0024 - Clinical Kinesiology and Anatomy, 6th Ed.
     e. CLINS 0025 and 0026 - Measurement of Joint Motion: A Guide to Goniometry, 5th Ed

NOTE:  

• The Statement of Need has the CORRECT ISBN numbers.
• This Amendment corrects the ISBN numbers in Attachment 6 - REVISED Contractor's Revised Quote Submission Spreadsheet 8 JAN 2026
• This Amendment corrects the ISBN numbers in Supplies or Services & Prices or Costs in Solicitation FA301626Q0013.

2.  Include Atch 6 - REVISED Contractor's Revised Quote Submission Spreadsheet 8 JAN 2026 with corrected ISBN numbers.

3.  Include Attachment 7 - Amendment 4 detailing the ISBN number changes as stated in paragraph 1 above.

4.  Correction from Amendment 3:  The quotes are due FRIDAY, 9 January 2026 at 8:00 am CST.

The purpose of Amendment 3 is to:

1. Extend the QUOTE due date as follows:

          a.  FROM:  Thursday, 8 January 2026 at 10:00 am CST
          b.  TO:  Thursday, 9 January 2026 at 8:00 am CST

       2. Correct ISBN numbers in forthcoming Amendment 4 (will be issued later today) in the solicitation and in Attachment 6.

       3.  Contractor can email acknowledgement of Amendment 3 to jill.van_plew@us.af.mil AND umar.thomas.1@us.af.mil by filling out: BLOCK 8 (NAME AND ADDRESS OF CONTRACTOR and CAGE CODE) BLOCKS 15a, 15b, and 15c

Amendment 2:

1.  The purpose to Amendment 2 for Solicitation FA301626Q0013 is to revise the solicitation CLIN structure and provide the following attachments:

• Attachment 3 - Amendment 2
• Attachment 4 - Government Responses to Contractor Questions

2.  Correct Delivery and Performance Text

3.  Extend the Quote Due Date to Thursday, 8 January 2026 at 10:00 am CST

The purpose to Amendment 1 for Solicitation FA301626Q0013

1. Add Attachment 2 -  Amendment 1 for Solicitation FA301626Q0013 which corrects the language on Page 43 of STANDARD FORM 1449  – Evaluation Factors for Award FAR 52.212-2 Evaluation – Commercial Products and Commercial Services 
2. Add text to Section F - Deliveries or Performance
3. Contractors shall acknowledge Amendment 1 in accordance with the instructions.
