DIR or its designee intends to contract with the Successful Respondent to provide a Survey and Forms Platform SaaS tool for forty (40) seats or licenses with the option to add in five to ten increments. DIR utilizes SurveyMonkey throughout for internal and external surveys. Surveys and forms are used to capture improvements, feedback, and report concerns. Responses are then able to be turned into graphs, charts, and allow metrics to be developed from the information collected. Respondent shall provide SaaS subscription licenses for the Licensed Software listed on the Pricing Table. The licenses must include telephone support, authentication, integrations, and a customer support. Each Licensed Software seat must include unlimited responses and access to all platform features including advanced branching, piping and multilingual support. Any catalog, brand name, or manufacturer’s reference used in the solicitation is descriptive only (not restrictive) and is used to indicate the type and quality desired.
