Contractor shall ensure consistent and fair monitoring of the State Centers including the provision of comprehensive monitoring activities, onsite monitoring, oversight, development and implementation of training processes, technical assistance, and implementing quality improvement and assurance systems. Contractor shall monitor the Centers in compliance with HHSC requirements and applicable terms of the Amended Agreement, including but not limited to: A. Conduct on-site visits to the Centers as requested by HHSC’s SSLC State office team; B. Aid the Centers in the establishment and maintenance of interrater agreement between monitors who evaluate center compliance with quality service review (QSR) tools; C. Collaborate with SSLC State office to identify Center trends or issues; D. Assist the Centers in identifying and planning for patterns, priorities and areas needing to be addressed; E. Create quality measures for resident outcomes and overall Center performance that align with the requirements of the Amended Agreement and move the Centers toward industry best practices; F. Assess the Centers’ data systems for reliability and validity through oversight and onsite monitoring services; G. Provide data-based monitoring system recommendations to ensure staff can be proactive and responsive to resident health and safety needs; H. Conduct independent analysis of data and information to identify patterns and areas for improvement in areas including, but not limited to, resident restraint, allegations and confirmations of abuse, neglect and exploitation, aggression between peers, and other incidents that may lead to negative resident outcomes; I. Development and implementation Center-specific training processes for staff including person-centered thinking and planning based on industry best practices;
