When completing an initial evaluation, the Contractor/Treatment Provider shall conduct: (1) a clinical interview, (2) a risk assessment, and (3) a psychological or physiological assessment. Any other techniques or tests shall be approved by the Program Supervisor prior to the Contractor/Treatment Provider conducting the evaluation. The interview for subsequent evaluations conducted by the Contractor/Treatment Provider is not required to be a clinical interview. The Contractor/Treatment Provider may utilize the following assessment tests for the purposes of an evaluation without requesting approval: 1. Personality Assessment Inventory (PAI); 2. Multiphasic Sexual Inventory (MSI-II); 3. Minnesota Multiphasic Personality Inventory (MMPI-2 and MMPI-2RF); 4. Millon Clinical Multiaxial Inventory (MCMI-III); 5.
