The Harlandale Independent School District (“HISD” or “District”) seeks to provide Physical Therapy “PT” (consult and direct) and evaluations to students with disabilities who ARDC deems appropriate. The District seeks qualified and licensed PT service providers to implement children’s PT individualized educational plans, conduct evaluations/eligibility per District and Federal Educational guidelines, and attend meetings with multi-disciplinary teams to discuss such services. PT providers must follow District standards for assessment/evaluation/eligibility and if eligible, develop individualized educational plans for those students eligible for services. PT provider must comply with timeline for services per federal law. Completed evaluations and documentation must be submitted prior to billing.
