Offeror shall: Accept Parkland’s patient referrals for services to indigent persons diagnosed with ESRD or AKI who are qualified to participate in Parkland’s Patient Financial Assistance program (“PFA”) based on the following: 1. Eligibility, Referral, and Admission Parkland and Offeror agree that Patients must meet the following criteria (“Eligibility Criteria”) to receive Services funded by Parkland pursuant to this Agreement: (1) Based on the information reasonably available to Parkland, Patient must be ineligible for participation in a federal healthcare program from which reimbursement for the Services might otherwise be available; (2) Based on the information reasonably available to Parkland, Patient does not participate in any private health insurance plan that covers the Services; (3) Patient will meet the then-current eligibility criteria for Parkland’s Patient Financial Assistance program (“PFA”) based on documented financial need and other criteria set by Parkland in its sole discretion; (4) Patient will have a documented diagnosis of ESRD or AKI and a treating physician with privileges at the applicable Dialysis Facility who has agreed to follow Patient...
