Specifications include, but are not limited to: Diabetes Self-Management Education refers to the comprehensive program helping individuals with diabetes learn how to manage their disease and be as healthy as possible. The primary goal of diabetes education is to provide knowledge and skill training, as well as help individuals identify barriers, facilitate problem-solving and develop coping skills to achieve effective self-care management and behavior change. The program includes Education Services provided to TWC customers including an initial and post training assessment of the customer’s ability to independently manage their diabetes at home, in the workplace, or assesses their ability to participate in intensive blind rehabilitation training such as Criss Cole Rehabilitation Center (CCRC) training or mini trainings. One-on-one training is conducted by the provider to better prepare a TWC customer to make informed choices about diabetes management; and help the customer develop the confidence and skills to implement choices, and to control their diabetes.
