Vendor is to complete pricing form attached and to include all costs associated with the services as stated herein including labor, materials, preparation, equipment, delivery, chemicals, and PPE for all locations. Unit pricing must be all inclusive. No additional fees will be permitted by Fort Bend County. 22.1 The number of cylinders throughout the twenty (22) EMS stations may vary depending on call volume. 22.2 All cylinders must contain medical oxygen only and be tagged; FULL, IN USE, or EMPTY. In the event it is determined through testing that a cylinder contains other matter, the vendor accepts full responsibility including but not limited to repairs to EMS unit, cost of testing and patient care. 22.3 Cylinders are transported and used in a horizontal and vertical position in the emergency response vehicles and are in a cradle to secure them as allowed in Compressed Gas Association, Inc. documentation CGA G-4 1996 (4.2.13). 22.4 Two day turnaround, delivery & pick up for hydrostatic testing. 22.5 Next day delivery on all oxygen cylinder orders. 22.6 Closed Loop RFID tracking on all oxygen cylinders. 22.7 Vendor must label each cylinder with lot or batch number, expiration date and test for purity (99.5% minimum). 22.8 All prices are F.O.B. Fort Bend County. 22.9 When term of contract expires and if incumbent is NOT awarded, the incumbent will be responsible for coordinating with Fort Bend County EMS to collect cylinders.
