1. Dallas Independent School District is seeking proposals from qualified vendors who are Federal Motor Carrier Safety Administration (FMCSA) Certified. Services required from vendor are the administration of medical examinations. 2. Proof of certification is required to administer examinations at multiple district designated locations and/or vendor's facilities. Medical examinations will be required for both pre-employment and post-employment services. 3. The district reserves the right to add or remove employees from the medical examination schedule at each department location, if it remains within the designated mass medical schedule. 4. Vendor must be available for week-long periods to conduct both Department of Transportation renewals and non-Department of Transportation renewals.Vendor must have the capacity to handle up to 200 appointments a day during the renewal periods throughout the year. 5. Vendor must also provide clear guidelines and procedures for employees who do not pass their medical examiner's recertification. 6. Vendor must provide medical examiner's certifications that is valid for no more than twelve months. 7. Vendor must provide online access to a medical examination database that will allow viewing and modification of testing participants.Vendor must provide monthly, quarterly, and annual statistical reports indicating number of district employees examined by type of examination and other relevant reports as requested by Student Transportation Services and Human Capital Management (HCM). 8. Vendor must provide written protocols and procedures for conducting medical examinations, either at the vendor's office location or Dallas ISD sites. Additionally, provide qualification training and proficiency demonstration documents for all technicians. 9. The vendor must provide documentation, including past records, staff count, and available resources, to substantiate and demonstrate their capacity to handle the required service volume. 10. Examination Requirements: Vendor must maintain facilities and personnel adequate to the performance of services agreed to by Dallas ISD. Vendor must maintain trained and certified personnel qualified to perform services provided.
