CLINIC SERVICES City of Longview Primary Care to include but not limited to: o Minor acute illness evaluation and available treatment for minor illness such as cough, earaches, fevers, headaches, and sinus infections. o Chronic illness evaluation, which includes evaluation, diagnosing, treating, and managing typical chronic illness that are handle at the Clinic, such as blood sugar, blood pressure, cholesterol, BMI, and tobacco cessation. o Minor Medical Procedures that are within the Health Professionals’ scope and abilities and that can be performed at the Clinic, with available medical equipment, and during operation hours. Health Assessments for Clinic Eligible Participants. Assessment is comprised of (a) blood draws and data collection for each participant (b) standard laboratory work (c) individual report for each participant accessible via mobile app and secure portal (d) aggregate report to plan administrator. Labor (Health Professionals working at each clinic) Temporary coverage during Health Professionals paid time off Account and clinic management (Director of client & Clinical Services, Client Manager and Clinic Manager) Marketing (electronic direct marketing materials, health education library, wellness newsletters, etc.) Technology (electronic medical records, analytics, patient portal, enterprise data warehouse, mobile applications, scheduling systems, etc.) Management Reporting and Analysis: o Monthly, quarterly & annual reports on health center activity, population health, operational costs, patient satisfaction and return on investment. Recruiting Health Professionals Quality and compliance (AAAHC – compliance standards, NPS patient feedback surveys, SOC 1 & SOC 2 or equivalent complaints) Insurance (general and professional liability) Travel and Meals Dues / subscriptions / memberships Training / continuing memberships
