- Provide an intervention to an older individual based upon the principles of State approved Evidence-Based Disease Intervention (EBI) programming: • Demonstrated through evaluation to be effective for improving the health and wellbeing or reducing disease, disability and/or injury among older adults • Proven effective with older adult population, using Experimental or Quasi-Experimental Design • Research results published in peer-reviewed journal • Fully translated in one or more community sites(s) • Includes developed dissemination products that are available to the public Contractor must: • Deliver either in-person and/or virtual health workshops, as required by the curriculum, in the Area Agency on Aging’s (AAA) 12-county region • Deliver workshops approved by the AAA and be on the National Council of Aging (NCOA) top tier Evidence-Based Practice (EBP) list: o accessible via the following website: https://connect.ncoa.org/older-americans-act-title-iii-d; and o in the attachments section of this RFQ, entitled FY2025 approved Evidence-Based Intervention Program • Be trained and certified to lead workshops • Be responsible for their own training costs and/or staff • Have two (2) trained instructors to lead workshops as required by the curriculums • Provide own transportation
