Health Maintenance Services include one or more of the following activities: • Medical treatment by a health professional • Provision of medications, nutritional supplements, glasses, dentures, hearing aids or other devices necessary to promote or maintain the health and/or safety of the older individual. Note: this also includes the provision of dosage alert systems and the purchase of software, technical support, and materials that connects eligible older individuals to free or reduced cost prescription medication services. Health Maintenance programs encourage healthy behaviors, reduce the risk for chronic and preventable disease, and maintain or improve the functioning of older adults. Health Maintenance is the provision of services, prescription medication, and/or assistive devices, which will prevent, alleviate, and/or cure the onset of acute or chronic illness; increase awareness of special health needs, and/or improve the emotional well-being of an older individual. The Area Agency’s role is to coordinate resources, where none exists, to create a way to fill gaps in services. Therefore, to prevent duplication of existing community resources, the Area Agency on Aging will make every effort to purchase services for which no other resource can be identified. The outcome is to increase the level of health and wellness of those persons aged 60 years and older by the utilization of community-based initiatives and the coordination with health care providers
