The required services to be provided are General Dental/Restorative Services for the residents at the Abilene State Supported Living Center, Services to be provided include on-facility and offfacility dental exams, consults, x-rays, provides general dentistry/restorative services including all preventative procedures, tooth extractions, surgical and non-surgical root canal therapy, direct and indirect restorations, fixed and removable prosthetics/devices, dental implant restorations, deep scaling root planning, crown lengthening, biopsies, and pathology evaluations for the dental clinic at ABSSLC in order for us to provide quality dental care for the individuals comprehensive overall health. Contractor shall provide dental exams, consults, x-rays, provides general dentistry/restorative services including all preventative procedures, tooth extractions, surgical and non-surgical root canal therapy, direct and indirect restorations, fixed and removable prosthetics/devices, dental implant restorations, deep scaling root planning, crown lengthening, biopsies, and pathology evaluations for the dental clinic at ABSSLC in order for us to provide quality dental care for the individuals comprehensive overall health 2. Contractor shall provide on ABSSLC facility services as needed before and after surgery. 3. Contractor shall provide services to ABSSLC clients including, but not limited to, providing and reviewing annual exams, evaluations, consultations, and specific dental treatment as appropriate to the residents(s) on campus as needed. 4. Contractor shall agree to maintain, clean, and sterilize all dental equipment as needed including all preventative maintenance. 5. Contractor and associated personnel shall enter findings, recommendations, and treatment summary onto a hard copy consultation generated by ABSSLC personnel or provide legible and signed clinical notes. 6. Contractor shall enter findings into the electronic medical record by ABSSLC staff. A copy will be provided to the patient’s Treatment Team for inclusion into the Treatment Plan as needed. 7. Contractor shall clinically oversee the work of the ancillary personnel from Contractor’s office. 8. Contractor shall be supervised by the ABSSLC Dental Director and interact with ABSSLC Dental Director and Clinical Dentist in a cooperative manner and will communicate with each as necessary.
