Specifications include, but are not limited to: The Contractor shall provide an annual full-service plan contract, including preventive maintenance and as needed repair services, for one (1) bioMérieux EMAG® instrument, serial number IM03135. Services shall be performed in accordance with manufacturer required maintenance services and inspection schedules. Annual service agreement shall include, at minimum, one (1) scheduled annual preventive maintenance and inspection, as needed preventive maintenance, unlimited repair services, non-consumable parts, labor, emergency on-site and remote services, technical support, and remote support. All parts and materials shall be provided on an exchange basis. All parts shall be without defect at time of installation and be installed per manufacturer specifications. Contractor shall provide a toll-free phone number for telephone support services available twenty-fours (24) hours per day, seven (7) days per week including holidays. Contractor shall perform remote maintenance and support including, at minimum and as applicable, diagnosis and repairs. Contractor shall schedule service personnel, for remote support services or in-person support services, as applicable, upon notification from the Agency. Contractor shall provide a detailed service report including, at minimum and as applicable, date, first and last name and title of technician performing services, diagnosis, resolution, summary of services performed. Equipment may be added or removed during the contract term at the sole discretion of the Department of State Health Services.
