The Successful Bidder will provide automated, internet-based access to a comprehensive employment and income verification service. The Bidder must maintain a database of employment and income information furnished directly by a national network of employer clients. The service will provide: a. Employee Information: Customer's full name and Social Security Number. b. Employer: Name, address, EIN, and wage garnishment address. c. Employment Status: Status of employment (e.g., active, inactive). d. Employment Dates: Most Recent Start Date and Termination Date (if applicable). e. Job Details: Total Time with Employer and Job Title. f. Pay Details: 1. Rate of Pay 2. Pay Frequency (e.g., weekly, bi-weekly) 3. Average Hours per Pay Period 4. Pay Cycle g. Income Summary: Up to three (3) years of year-to-date gross income details. h. Pay Period Detail: Up to three (3) years of detailed pay period information. i. Benefits: Medical and dental insurance information. j. Other: Workers’ Compensation information.
