Specifications include, but are not limited to: CDC Strategy A1 - 1. Conduct a capacity assessment to identify a local partner organization to become an Association of Diabetes Care and Education Specialists (ADCES)-accredited/American Diabetes Association (ADA)-recognized organization to implement diabetes selfmanagement education and support (DSMES) services. Provide technical assistance to the selected organization. Priority should be given to organizations in rural areas with a high prevalence of diabetes, limited access to DSMES services, and that serve nonHispanic/non-Latino Black persons and Hispanic White persons (priority populations of focus). 2. Develop a minimum of one complementary diabetes support program service in collaboration with the Texas Department of State Health Services (DSHS). The program and services should be within the Public Health Region (PHR) where there is a high burden of diabetes among non-Hispanic/non-Latino Black persons (priority population of focus) CDC Strategy A5 - 3. Support a minimum of three healthcare organizations to expand existing CDCrecognized lifestyle change programs (LCPs) to new locations and new times to increase program accessibility for priority populations of focus with low enrollment rates. New locations where diabetes prevalence is high, but the availability of LCPs is low will be prioritized.
