a) The Contractor shall furnish all labor, tools, transportation, equipment, materials, and supplies, as necessary, to perform the services and/or provide the goods, as required. b) The Contractor shall be responsible for supervision of its employees and Subcontractor employees, together with clean up and proper disposal of any site work waste. c) Contractor must hold a DDS or DMS degree in dentistry. d) All services performed will meet all requirements of the Texas State Board of Dental Examiners to provide dental services. The Contractor shall comply with all federal, state and local laws, statutes, ordinances, rules and regulations, and the orders and decrees of any court or administrative bodies or tribunals in any matter affecting the performance of the Contract including, if applicable, workers’ compensation laws, minimum and maximum salary and wage statutes and regulations, and licensing laws and regulations. e) Certify that license, permit or certificate has not been revoked by any Health and Human Services agency or public safety and criminal justice agency.
