The required services to be provided are Dental Services for the inpatient population at the San Angelo SSLC (SGSSLC) Services to be provided at the SGSSLC dental clinic. Services included but not limited to are dental exams, consults, x-rays, general dentistry/restorative services including all preventative procedures, oral surgery including extractions (including 3rd molars), simple fillings, retained root procedures and provides biopsies and pathology evaluations. A. Comply with HHSC State Supported Living Center/Facility Uniform Terms and ConditionsVendor, which are incorporated as part of this Purchase Order as Attachment A. B. Provide HHSC with the following service(s) to patients of the Facility: • Provide all oral surgery, extractions (including 3rd molars), implants, evaluations, retained root procedures and provides biopsies and pathology evaluations. • Provide dental exams, consults, x-rays, general dentistry/restorative services including all preventative procedures, tooth extractions, surgical and non-surgical root canal therapy, direct and indirect restorations, fixed and removable prosthetics/devices, dental implant restorations, deep scaling root planning, crown lengthening, biopsies, and pathology evaluations. • Licensed Dentist will provide services including, but not limited to, providing, and reviewing annual exams, evaluations, consultations, and specific dental treatment as appropriate to the client(s).
