The South Texas ISD is interested in selecting one or more primary vendors, offering the lowest net price for dot and non-dot employment physical screening & laboratory testing services. The catalog pricing should include, but not be limited to, Dot, Non-DOT, student drug and alcohol testing services, on-site bus driver physicals, student athletic physicals, urine drug test kit, on-site breath alcohol and drug testing collections, 3 minute 5 panel, or customized panel- FDA approved drug test and breath alcohol testing for immediate results, SAMSHA 5 panel drug test or customized panels, transportation of specimen, collection fees, supplies, chain of custody, medical review officer services, computer generated random selection random record maintenance, substance abuse professional services, statistical reports, employee qualification reports, employee record setup and administration off-site, positive specimen documentation and storage, blind specimen maintenance, supervisor training, on an as-needed basis.
