The intention/purpose of this Request for Proposal (RFP) is to solicit responses from firms qualified to perform contracted services to include, but not limited to, Visually Impaired and Orientation and Mobility Services, Occupational Therapy, Physical Therapy, Speech-Language Pathology, Diagnostician/School Psychology Evaluation Services, Itinerant Services, Language Interpreters, Sign Language Services, Medical Assessment Services, and other Related Services, for the Clear Creek Independent School District. Therapy materials, supplies, equipment, evaluation kits, and protocols will be provided by the schools. Therapists will have access to network connectivity and printers. Contracting agencies can bill for both direct and indirect treatment time (paperwork, meeting, teacher consultations, etc.). The district does not pay/reimburse for mileage traveling between school campuses. Service provider(s) must clear a criminal background check through Clear Creek ISD. Service Providers shall provide a copy of the following: • Current Proof of Professional Liability Insurance –If Required • Verification of Tax-Exempt status, if applicable • Proposed Fees to be charged to the District • Notice: If you are using the service of TRS retirees, the employment of such retirees is subject to the laws and rules governing employment after retirement and must be reported each month.
