Specifications include, but are not limited to: PATH Clients: PATH homeless services are available to literally homeless and those individuals at imminent risk of becoming homeless who have serious mental or co-occurring substance abuse disorders. Literally homeless people are those who are actually without shelter, except for emergency shelter provided by organizations such as the Salvation Army. Literally homeless people are most frequently found in emergency shelters or on the streets. Those individuals at imminent risk of becoming homeless commonly includes one or more of the following criteria doubled-up living arrangement where the individuals name is not on the lease, living in a condemned building without a place to move, arrears in rent/utility payments, having received an eviction notice without a place to move, living in temporary or transitional housing that carries time limits, being discharged from a health care or criminal justice institution without a place to live. People who seek to enter PATH services from a crisis unit or emergency respite care may be considered marginally homeless. 2. PATH Services: The PATH program consists of the following services: a. Alcohol or Drug Treatment. In the event respondent is not a licensed substance abuse treatment provider, these services may be subcontracted b. Assertive Community Treatment (ACT) c. Case Management d. Community Mental Health Services e. Costs associated with matching eligible homeless individuals with appropriate housing situations f. Habilitation and Rehabilitation Services g. Housing Services as specified in Section 522 (b) (10) of the Public Health Service Act, including: i. Minor Renovation ii. Planning of Housing iii. Technical Assistance in Applying for Housing Services iv. Improving the Coordination of Housing Services v. Security Deposits. h. Mental Health Crisis Services i. One-time rental payments to prevent eviction j. Outreach k. Referrals for primary health services, job training, educational services, and relevant housing services l. Screening and Diagnostic Treatment m. Service Coordination n. Staff Training, including the training of individuals who work in shelters, mental health clinics, substance abuse programs, and other sites where homeless individuals require services o. Supportive and Supervisory Services in Residential Settings and p. Other appropriate services determined by DSHS. Each awarded contractor shall provide PATH services according to the Contractors DSHS-approved PATH work plan.
