Specifications include, but are not limited to: • Meet with and advise HCPH on a routine basis set at the discretion of the department to: o Discuss evaluation design, methods, data governance, and tools to address the key evaluation questions. o Develop additional research questions and analysis. o Build productive, long-term relationships with HCPH and its stakeholders that are essential in making a meaningful impact. o Provide regular progress reports on evaluation status. o Present emerging findings to CHVPS and HCPHS leadership. • Design and implement a comprehensive evaluation for each of the CHVPS programs that includes but is not limited to: o An approved logic model for each program. o A systematic review process of the programs and practices of the CHVPS division. o Mixed-methods (quantitative and qualitative) evaluation customized to each program. o A formative evaluation that assesses the implementation of program components and outcomes and provides recommendations for improving the implementation and model by tracking outputs and outcomes. o A summative (impact) evaluation that assesses whether the interventions (all CHVPS programs) cause specific changes, outcomes, and benefits for the participants. o A cost-benefit or cost-effectiveness analysis that relates the program's costs per participant to its financial outcomes, health outcomes, and all other assessed outcomes. • Collaborate with HCPHS and other stakeholders to collect data. • Coordinate meetings with authors of evidence-based tools to be used for evaluation purposes. • Assess current gaps and challenges in the delivery of all services. • Tailor evaluation designs to each program's characteristics, goals, and activities to ensure HCPHS attains the data required to evaluate program success.
