To support and implement care management services including chronic care management and remote patient monitoring. The vendor will provide support throughout the implementation process, from start to finish and beyond. Also, align with our commitment to improving quality and outcomes for our patients and community. Our care management goals align with the Quadruple Aim – a focus on improving health outcomes, improving patient experience, reducing costs, and provider satisfaction. We are seeking a vendor partner to provide technology, care management personnel, and best practices to support the implementation and ongoing service delivery of chronic care management and remote patient monitoring. The proposal should support continuous improvement and improved patient outcomes as well as aiding in the long-term goals and evaluation of establishing a Texas A&M Health hybrid or insourced care management team and services.
